| You will be asked to agree to the
following conditions when submitting an entry to Art Works
in Mental HealthTM: |
- I agree that I continue to own my work of art at
all times but allow the organisations specified on
the About
Us page of this website and their agents
to use, display, copy or reproduce it in any form
or media for the following purposes:
- An exhibition visiting galleries in the UK.
- Exhibition brochure
- Postcards to be distributed to the public
- Art Works in Mental HealthTM website
- Art Works in Mental HealthTM advertisements on
public transport
- Posters for healthcare premises
- Display panels showcasing other sponsored medical
or education services in mental health
- CD-ROM containing screensavers for healthcare
professionals, mental health art groups and other
mental health organisations
- Inclusion in 'Reflections' magazine published
by the Mental Health consultancy Breakthrough
I understand that I will be identified as the artist/creator
of the work of art unless I have asked not to be identified
on the application form.
I understand that, if selected, my work of art will
be showcased in order to promote my artistic ability
and raise awareness of arts and creativity in mental
health. The art will be showcased for non-profit purposes
and I accept that I will not receive payment for its
use as described.
I understand that Art Works in Mental HealthTM may
not be able to showcase my work of art.
Data protection. I understand that my personal details
will be recorded on a database by Pfizer Ltd for use
by Pfizer and mental health organisations working
with it within and outside the EU. Such details may
be used to keep me informed of mental health matters
or other health related initiatives. Such details
can be removed from the database at my request.
